Events

I have been informed of the activities related to my child’s registration at North Bay Community Church and hereby give consent for my child to participate in these events and for pictures to be taken and/or posted on our website and/or Facebook pages. I recognize that there are inherent risks in any activity and recognize that staff and volunteers will make every reasonable effort to provide for the safety of the students, and I agree to hold harmless all staff and volunteers affiliated with this programing/organization. In the event of a minor injury, I give consent for staff or volunteers to administer basic first aid. Further, in the event of an emergency, I provide authorization for 911 to be contacted and for the administration of emergency care in my absence. I understand that as the parents/guardians, I/we will be notified as soon as possible of the circumstances and steps taken in the emergency. This form is considered good for 12 months from the date signed, unless additional written notice is given.
